At Diamond Hearts, we have a profound sense of fulfillment in the knowledge that we are helping our clients to live a more fulfilled life irrespective of their situations in the comfort of their homes.
We love the fact that we strive to positively impact other people’s lives and care for them as we would want to be cared for. We are passionate about caring for people.
What inspired you to start your own business?
Our inspiration was as a result personal experience when a loved one was diagnosed with a terminal disease. The level of care received was not good enough and we do want other families to go through the same experience. Therefore, Diamond Hearts Home Care Ltd was set up to provide to our clients the level and quality of care we would want for ourselves, family and loved ones, offering them support to live independently, in the comfort of their homes.

What changes have you made to keep your customers safe from Covid-19?
All our staff are regularly trained and updated on the latest Covid-19 guidelines and policies and are appropriately equipped with the relevant PPEs.
Dementia Care
We understand how difficult, challenging and stressful it may be when one is diagnosed with dementia; and is unable to enjoy life the same as before the diagnosis. That’s why Diamond Hearts team is here to take away the anxiety and stress, and help you maintain active social life as much as possible with little or no disruption to your personal routines thereby, making you feel better about yourself.
Having a balanced diet is an integral part of healthy lifestyle. The fact that dementia patents does not eat sometimes, does not mean that he/she do not want to eat. However, as a result of suffering from dementia, one may not realise that he/she is thirsty or even hungry. Continued lack of eating and drinking may worsen the effects of dementia for e.g., confusion and paranoia. That’s why our experienced team members will set aside enough time for our clients' meals, to offer them foods we know they like and provide fluids in clear glasses, based on their personalised care plan.
We recognise how incontinence (urinary or bowel) affects people with dementia. At Diamond Heart, we undertake careful assessment of our client’s situations and living environments, in consultation with the relevant stakeholders including families and relatives; and make adjustments, where necessary. Such little adjustments, as putting a toilet sign, keeping the toilet door open, having a sensor lighting in the toilet, understanding 'unique' client's signs (e.g., fidgeting) that one may be in need of a toilet, will go a long way in improving our clients' conditions.
We therefore provide targeted assistance, to meet the various needs of our clients - dietary requirements, physical activities, therapies, support for challenging behaviours, etc. We also provide specialist care for our clients irrespective of the type or stage of dementia they may be living with.
Palliative Care
Death is inevitable, but how well and comfortable our clients live until the final moments is something they will always cherish. That’s why we manage our clients’ pains, symptoms and other circumstances so that, they are as comfortable as possible, especially within their last few months, while also providing physical, psychological and emotional support to their families and relatives, in order for them to spend the time they have left as they deem fit. In other words, we take a holistic approach when delivering palliative care to our clients.
We also support end of life clients who are in the last few weeks/days of their lives due to life-limiting conditions like cancer, heart failure, lung disease or old age, giving them the best quality of life possible. We take special care in personalising our clients’ care plans, paying attention to what really matters to them, even as they approach their last moments.
Personal Care
We understand how important your personal routines are to you. That's why the support with personal care we provide to our clients is delivered with utmost consideration to your individual needs and personal routines; giving them the choice and control over those things that really matter to them.
We provide personal support and supervision for our clients' daily routines, tasks and personal hygiene, for e.g., toileting, bathing/showering, applying lotions/creams, oral hygiene, make up, shaving, etc.
Irrespective of physical or mental health conditions that our clients may have, we will help them make decisions about managing them, with every agreed outcome making sense to them, enabling them live life the way they want to.
Respite Care
We all need a break at some point, don’t we? Taking a break to look after oneself especially when caring for loved ones, helps one to get energised and refreshed, and stops the feeling of exhaustion and rundown; and at the same time, one's loved ones still gets the required care.
Our team also understand how looking after one's wellbeing and focusing on the things that one, as an individual, enjoy doing will help reduce the pressure that comes with caring for loved ones, while maintaining a balanced life.
Our experienced team members will keep your loved ones company and make sure they do not hurt themselves, while you take some time off, to rest and relax, go on holiday, shop, attend to appointments, work and attend to other pressing needs that matter to you.
We do not only provide respite care for family members who care for their loved ones, we also act as companions to their loved ones in the comfort of their homes, while their carers take the much-needed time off.
Supportive Care
When providing supportive care, we also focus on the symptoms and pain management; and how our clients feel about meeting their nutritional, physical, emotional, psychological, social, informational and practical needs.
We work collaboratively, with other health care professionals to provide additional layer(s) of care, to our clients and their families who are facing serious illness or life limiting conditions.

Grocery Shopping/Social Hours
When providing supportive care, we also focus on the symptoms and pain management; and how our clients feel about meeting their nutritional, physical, emotional, psychological, social, informational and practical needs.
