A great website should be easy-to-use with clean code and a user-centred flow. This gives users the experience they expect and deserve.
What questions might you ask a client when starting a new project?
It depends on the project, but I'll usually ask what purpose your end product has and to what demographic.

I prefer a client-focussed process, working together with them as a team to ensure the best possible product is delivered, to the highest standards.
What information do you need from a client before you can start work?
It depends on the specific job, but, at a minimum I'll need a brief written concept of the end product you wish to create and a deadline for completion of the project.
