Services

Our Home Care service provides at home assistance from a few hours a week to multiple visits per day. We can assist you or your loved ones with all aspects of personal care. We focus on a Person-Centred approach, promoting independence by encouraging and involving individuals in their own care to the best of their abilities.

We can provide services in:

• Body Wash – Full or partial to your requirements
• Bath / Shower – Including hair wash, if required
• Application of creams – After washing or multiple applications per day as prescribed by doctor or district nurse
• Dressing / Undressing
• Personal Grooming
• Oral hygiene care – Including dentures
• Continence Care – Including Catheter care, Stoma care and Incontinence pad changing
• Mobility Assistance – with or without mobility aids

SPECIALIST MOBILITY CARE PROVIDED BY TRAINED AND EXPERIENCED CARERS

Our specialist mobility and frailty care is founded on an expert approach to care delivery. All our dedicated carers are experienced in caring for someone living with the challenges of reduced mobility. Our gentle and compassionate carers encourage you to live as independently as possible and recognise the importance of maintaining control and choice in your life, with all the support you need. Regardless of your circumstances, if you have ever faced mobility concerns such as unsteadiness while walking or difficulties getting in and out of chairs, you may benefit from mobility help at home.

Dressing

Fastening buttons and zips, and tying shoelaces are simple processes to most, however, for many individuals, such things can become impossible and distressing tasks. If you need help getting dressed and undressed, you can rely on our support workers to provide assistance to make things easier and more comfortable for you.

Continence

We are able to support both your urinary and faecal continence whilst preserving your dignity and comfort. Your support worker can help you get to and from the toilet and, where required, will monitor your personal hygiene to ensure your health and comfort. This can include changing catheter and stoma bags, incontinence pads and sheets, and help with bathing and changing clothing.

The comfort we feel in our home is like no other. It is the place where we feel most relaxed and happy. However, as family members grow up and move into their own homes, you may find your house isn’t as busy as it once was and there aren’t as many people around to give you that extra hand when you need it.

Our companionship service gives you the support that you need to live independently, providing you with the assistance of a Healthcare Assistant or Support Worker at times to suit your convenience. Our Carers will provide company and conversation as well as providing support with household and social activities. Our companionship service is most suited to individuals who will benefit from having the reassurance that someone is there with them to offer support when it is needed.

It also provides peace of mind for family members who are unable to regularly visit, as they know their loved one is being cared for. Our service is tailored to your requirements and you can decide which activities you receive support with and when. Should you need to alter the level of care that you receive, we will be happy to arrange this for you.

Dementia is frightening and confusing for both people with the condition and their families and friends. Expert dementia care in the home can make the process of coping with dementia a little bit easier. People with dementia can remain in familiar surroundings while receiving the care and support they need.

We are dedicated to a bespoke, individualised person-centred approach to healthcare. We employ qualified healthcare staff who are available to work on a one-on-one basis where needed with the people we support.

Our professional care staff all receive regular training in mental health care. Many also specialise in caring for people with dementia. The dementia care provided by our staff is based upon our key principles of trust, empathy, and expertise. We aim to help the people we care for live as fulfilling and independent a life as possible. We recognise that to achieve this we also need to support the friends and family of the patient. As such, we are always available to discuss any concerns you may have.

We offer a bespoke approach to care in the home. Every step of the way we at Gran Care we provide the best care to the people we work with. The overnight care we offer is tailored to each individual’s needs and concerns.

The starting point for an overnight care plan is an initial discussion with you or a loved one. This is done through a friendly Gran Care assessor who will discuss your needs or that of a loved one. This initial discussion will form the basis of the overnight care plan. This information is provided to our highly trained care assistants who will deliver the support the service user requires.

Our staff work on a shift pattern to ensure that the people we work with are provided with the best care. We regularly assess and review the support we offer to the service user to ensure that it is the best it can be.

Live in Care is when care is provided in your own home by a career who lives with you. A fully trained carer will live in your home to provide a 24 hour, one to one care.

This is a good choice for those who do not want to live in care homes and want to live a more independent life. Live in care is available for adults and young adults. Our live in service provides a consistent one to one support tailored to your needs.

The career may help with the following, personal care, domestic tasks, companionship and all day to day activities.

Most of us would like to spend our final days and weeks in peace, dignity and comfortable surroundings. During these final days, we inevitably want to be surrounded by our family and close friends, to reduce the stress of unfamiliar surroundings such as a care home, hospice or hospital.

We can support people to return home from the hospital to enable the last days to be spent with home comforts and in the company of those closest to you.
