Services

Respite Care is there for you when your carer – a family member, friend or neighbour – has regular time away from your home. You’ll be cared for in your own home, continuing to do everything you usually do while your carer knows you’re in good hands.

Our care workers can take you out to social activities, walk your dog or just spend time with you.

When you choose respite care, our carers come prepared to slot into your normal routine, remembering your Right to Respect every time.

With Crisis Care, we can be there at your side within 24 hours if an accident or sudden medical condition means you need our help.

Live-in care provides constant, round-the-clock care from a dedicated carer who lives with you in your home, offering a helping hand at a moment’s notice. We understand that being supported by or looking after a loved one with care needs can sometimes be overwhelming. That’s why our live-in care is centred around your individual needs, routines and preferences.

With live-in care from Tokscare Solutions, you and your loved ones can focus on the most important thing of all: spending quality time together.

What's included in live in care?

Going out and about
We understand your life extends beyond your home, so we’ll support you there, too.

What goes into a complex support plan?

A complex care support plan will include all of the details relating to an individual’s health care needs, including any diagnosis that they may have, and the treatment required. Written in first person, a support plan is centred around the individual and their wishes, and will include important information such as their next of kin and other medical professionals involved in their care.

Dementia care is all about helping you or a relative to have the support they need to carry on living in their own home. Our carers always work to follow a routine that you’re used to and comfortable with, maintaining independence as much as possible, and always promoting the highest standard of care for the person they’re supporting.

Dementia homecare

Whether you just need someone to check on your loved one first thing in the morning and help with personal care or breakfast, or continuous live-in care to make sure they’re safe during the night, you can be assured that our fully-trained carers will be on hand to help whenever you need them. They can also assist with other areas, such as:

Medication support
Helping with mobility
Facilitating hobbies, activities, and social interaction
Preparing meals and tending to household tasks

While some receive palliative care at an inpatient hospice or residential nursing home, it is becoming an increasingly popular choice to get access to palliative care at home. Many individuals want to stay in an environment where they feel most comfortable as a way of managing emotional responses to their symptoms; it’s often referred to as hospice care at home.

Palliative care at home can involve:
Personal care and assisted living such as assistance with bathing, dressing and toileting
Continence care, whether it is changing continence pads or managing a stoma or catheter
Medication support including prompts or administering medication, even the more complex prescriptions
Support with pain management
Help moving around the home, whether it is gentle support or hoisted transfers
Assisted living through housekeeping duties such as vacuuming, dusting, changing beds or doing the laundry
Preparing meals to your tastes, and washing up afterwards
Caring for your pet including feeding and walking
Running errands to collect prescriptions or food shopping
Companionship and supporting your independent living at home

Visiting care is for anyone who wishes to remain independent in their own home, but requires a level of support to do so.

It may be that you have lost confidence following a fall or that you need extra support after a period in hospital. Or maybe your care needs have increased and you would like a little more support on a regular basis. Whatever your need, the extra help our home care services provide can make all the difference.

We believe in offering a truly quality service, providing the care industry’s renowned carer training and full ongoing support to each of our carers so they can give the very best level of care to you.

You don’t have to spend months, or even years, surviving life and pushing through difficulties on your own.

We support you through the emotional and psychological impact of the COVID-19 crisis, so you can bounce back stronger than ever.
