We offer free care assessments to help you determine if dementia care services are right for your loved one. Your loved one’s care assessment will be performed in the comfort of his or her home. During this assessment, we will outline the foundation for your loved one’s dementia care services.
We customize a care plan around your loved one’s lifestyle, preferences, and symptoms. We then match your loved one with a caregiver whose skills and strengths are particularly suited to your loved one’s care needs.

Having a carer come to visit you in your home can make a huge difference to your life, especially if you have difficulty walking or getting around. It can help you stay living independently in your own home.
Our home help service is aimed at supporting you or your loved one with those tasks around the home that you face challenges performing, and to ensure your safety.
We are committed to improving the welfare of our Service Users. These are some of the tasks we can offer help with:
• Housework
• Laundry
• Preparing light meals/meal delivery
• Companionship
• Running errands, e.g. prescription collection

You or your loved one may need night-time care. We provide night-time care, for both sleep-ins and waking nights where a fully trained carer is there to support you.
Given that everybody is different and each situation is unique our overnight care is tailored to you. With a dedicated night-time carer, help is available at any time, meaning that you or your loved one can stay in your own home – the place that is most comfortable.

Sleep-in:
The Carer sleeps at your property and is on hand if required. This option will suit if you don’t require regular support, but feel uncomfortable when left alone throughout the night. Anxieties and sleep deprivation can soon be improved with a sleep-in Carer there to support you. Having that extra night care at home can really help you remain healthy and happy. If assistance is needed more than twice during the night, this will be considered a waking night.

Waking night:
The Carer is on duty throughout the night. This support is best suited for you or a loved one if you/they wake up several times perhaps being disorientated or needing to go to the bathroom. If you wake at regular intervals during the night to change position due to injury, or if you have to administer medication at regular intervals, this is likely to be the best choice for you.
The waking night Carer is there at your side as and when you need them for the duration of the night, providing a high level of expert support.
Overnight care can ease yours or a loved one’s anxiety about being alone at night. But it also helps in many other ways, including:
• Support with administering medication through the night
• Assisting with toilet breaks
• Regular help for changing position in bed, particularly after an operation or injury

Hallmark healthcare service provides supported living services to enable younger adults between 16 to 30 years with challenging conditions to be able to live and develop fulfilling lives at home and in their local communities.

We provide person-centred support which enables each young person to grow and develop their skills and independence, ready for adulthood. In particular, we can support young people who have learning disabilities, autism, behaviours that challenge or other complex needs to live safely and as independently as possible. Our supported living care ensures better outcomes for young people and maintains their relationships with family and friends.

• Supported living enables young people to remain or to return home or to their own communities where they remain close to family and friends and also be able to continue using local facilities and services.
• Supported living allows young people to become more confident and independent which leads to greater self-esteem.

• Essentially, supported living ensures that young people with challenging behaviour fully prepare for adulthood, and move on to lower levels of support as and when the time is right.
Our Support model:
Hallmark healthcare service involves the young person in need of support, and those who know them well in the design and delivery of their support. We place each young person at the centre of decision-making.
We use person-cantered active support to ensure that each individual is actively encouraged and enabled to take part in activities and is able to grow and develop their skills and independence.
Our robust support plans are outcome-focused so that people progress to meet their short, medium and longer term goals. Young people benefit from a Key Worker who regularly reviews their support.
We adapt support to each young person’s daily activities, be that school, college, training or employment. We can provide additional support in the mornings, evenings and at weekends to enable each person to lead a full and varied life.
If needed, we can provide continuity of support to young people through to our adult provision.

We provide temporary relief for primary caregivers, enabling you to take a much-needed break from the demands of caregiving a sick, aging, or disabled family member.

Whether it’s for just a few hours a week or an extended vacation, seeking respite care can help ease the burden of family caregiving and help to relieve stress, restore your energy, and promote balance in your life.
