We do provide remote consultation and assessment, but our services are in home of the clients.

Our clients' safety is our top priority. All of our staff are fully vaccinated, they wear proper PPE, and masks while taking care of seniors.

Neighbourhood Care provides around-the-clock nursing services based on needs for a variety of medical conditions.

Personal care is such an important part of us all having our dignity and respect. In some cases, we require assistance to complete these tasks. Neighbourhood Care can help. These are just a few ways we can help.

• Assistance with personal care including showering, bathing, toileting, grooming, and dressing
• Uniquely tailored to your loved one’s personal hygiene needs and preferences
• Respite care for family caregivers during any time of the day as needed
• Behavioural management: sitter and caregiver services (at the hospital, health care facility, or at home)
• Assistance with pharmacy, medication refills, and ordering and delivering of medical equipment and aids
• Assistance with range of motion exercises and physical activity

Loneliness can lead to depression and an overall unhealthy state of mind. Neighbourhood Care does provide Companion Services to assist with keeping up social skills such as:

• Participate in activities such as board games, puzzles, entertainment and movies, and reading
• Escorted outdoor walks for sensory stimulation
• Spending time and engaging in meaningful activities and conversation
• Assistance with light meal preparations, food storage, running errands, and grocery shopping

Seniors who want to stay in their homes, do require assistance with home maintenance. Neighbourhood Care can help:

• Internal & External home supports and maintenance
• Assistance within and outside of the home:
o Seasonal clean-up
o Yard work
o General cleaning: laundry, vacuuming, sweeping, dusting, mopping, dishes, ironing, wiping down counters
o Organization of spaces and storage rooms
o Assistance with decluttering
o Light fixing such as changing of batteries, smoke detectors and alarms, lightbulbs, etc.

Overall wellness is important to good health. Neighbourhood Care offers Consultation on wellness & health such as:

• Providing regular updates to family members and emergency contacts
• Health consultations and accompanying client to medical appointments, follow-ups, and check-ups
• Note-taking and explaining of health care plan between third parties (doctors, interprofessional care providers), client, and family members for smooth communication and continuity of care
• Assistance with transition to adult lifestyle residence or LTC
• Elder fraud detection and prevention
• Up to 24-Hour, 1:1 support for monitoring of health, safety, and overall well-being

• Hospital Discharge services are available at an additional fee upon request, as individual components or as a full-service offering. Please confirm needs with the Neighbourhood Care Nurse for pricing
• Discharge Coordination by conducting an initial 360 meeting with the hospital Discharge Planner, Social Worker, or Discharge Nurse. o Receive discharge instructions o Transfer of any documents from the discharge planner to Neighbourhood Care Inc o Create a detailed Care Plan
• Home Visit to ensure the client home is properly prepared prior to the discharge o Full Coordination of Care services o Create list of any equipment needed o Discuss with authorized person responsible for the client o After approval make arrangements – order & delivery o Coordination and Delivery of Medical Equipment/ Devices
• Make arrangement for transportation on discharge day to home.
• Nursing and Personal Support Care including Medication Management Involves organizing and tracking medications to make sure that they are taken as prescribed and for any adverse effects as needed meet with Pharmacy/ Pharmacist discuss medications
• Monitoring Involves becoming familiar with the warning signs and taking appropriate action in a timely manner.
• Daily living activities to support being at home needs o Meal Preparation and Housekeeping Involves meal planning, shopping and preparation according to the care plan, other as needed
• Follow-up medical appointments are tracked, communication with other health care professionals and make any arrangements needed for transportation related to Physician Visit Support, or additional medical specialist appointments
• Medical visit note taking in the appointment with client if needed on behalf of client

Respite is such so important part of caregivers’ health and wellness. Caregivers are encouraged to take short periods of time off to rest and recharge while knowing everything will be all right. We like to think of that as peace of mind! In many cases, family caregivers and full-time paid caregivers need to take time for themselves, and at the same time, ensure the primary care needs and safety duties are still carried out in the same manner as the primary caregiver (s) would have done. For peace of mind, this temporary care for a short time period can be entrusted to Neighbourhood Care Services. Neighbourhood Care can help and can be part of your plan in the same manner as you would have provided care for you to take the time to do some of these things for yourself such as …… • A weekend trip to rest • A vacation to recharge • A visit to see other family members • When you are not well, some extra help • Be a “Snow Bird” spend the winter down south and we send you weekly updates • Attend important family events – Weddings, Funeral, Graduation …… • COVID – isolation if needed to protect the person in your care • Other unique plans based on individual family request.
