Supporting adults who have a diagnosis of severe and enduring mental health needs. Developing and maintaining daily living skills such as (but not exhaustive) development of domestic skills, effective budgeting and self management of finances. Establishing and maintaining meaningful and fulfilling Social structure. Identification and support for training, Education and employment interests and maintaining positive mental Health and wellbeing. Effective person centred approach to realistic goal setting and progress monitoring working with the STAR tool, time lines, WRAP and continues reviews and assessment. Initial assessment of service suitability, discussing in detail potential new clients in team meetings. Writing and chairing reviews a and discharge planning meetings. Self directed payment assessment, application and management. Supported people fund assessment, application and management. Risk assessment and contingency planning. Assessment, recommendation, implementation and management of financial apointeship through client finance. Ordering and dispensing medication and keeping accurate records in relation to this. Advocacy, supporting family and carers, accurte record keeping, working within company and legal policies such s confidentiality, data protection and lone working. To support adults with a diagnosis of enduring mental health needs to maintain mainstream tenancies. To support clients with a range of independent living skills such as cleaning, cooking, shopping, monitoring medication ordering and compliance, attending CPA/Psychiatry/consultant etc meetings and updating involved services as to any issues or concerns they may be. Keeping accurate case notes and personal information in line with policies such s confidentiality and data protection. General office duties and attending training identified and applied for by the line manager.
