Services

(PSR) services are designed to assist the individual with compensating for or eliminating functional deficits and interpersonal and/or environmental barriers associated with their mental illness. Activities included must be intended to achieve the identified goals or objectives as set forth in the individual’s individualized treatment plan. The intent of PSR is to restore the fullest possible integration of the individual as an active and productive member of his or her family, community and/or culture with the least amount of ongoing professional intervention. PSR is a face-to-face intervention with the individual present. Services may be provided individually or in a group setting. A minimum of 51% of a PSR’s contacts must occur in community locations where the person lives, works, attends school and/or socializes.

(CI) services are provided to a person who is experiencing a psychiatric crisis and are designed to interrupt and/or ameliorate a crisis experience, via a preliminary assessment, immediate crisis resolution and de-escalation and referral and linkage to appropriate community services to avoid more restrictive levels of treatment. The goals of CIs are symptom reduction, stabilization and restoration to a previous level of functioning. All activities must occur within the context of a potential or actual psychiatric crisis. CI is a face-to-face intervention and can occur in a variety of locations, including an emergency room or clinic setting, in addition to other community locations where the person lives, works, attends school and/or socializes.

(CPST) are goal-directed supports and solution-focused interventions intended to achieve identified goals or objectives as set forth in the individual’s individualized treatment plan. CPST is a face-to-face intervention with the individual present; however, family or other collaterals may also be involved. A minimum of 1% of CPST contacts must occur in community locations where the person lives, works attend school, and/or socializes.

Is provided to assess, monitor a consumer’s status in relation to treatment with medication, instruct the recipient, family, significant others or caregivers of the expected effects of therapeutic doses of medications, administer prescribed medication when ordered by the supervising physician as part of a mental health rehabilitation plan which is inclusive of additional rehabilitation services and supports.

Is a therapeutic intervention involving the client and one or more of his/her family members? The primary goal of the service is to help the client and family improve their overall functioning in the home, school, work, and community settings. The goal is accomplished by helping the client and family increase effective coping mechanisms, healthy communication strategies, constructive problem solving, and increased insight into the client’s difficulties and the impact on the family.

Are verbal interactions between the counselor/therapist and the consumer that are brief, face-to-face, and structured. Individual intervention, (Youth) is a range of professional delivered therapeutic strategies provided individually and face-to-face to the consumer. The purpose is to rehabilitate and restore the consumer to an optimal level of functioning and to reduce the risk of more restrictive treatment intervention.

Services are designed to assist children who, are or will be, transitioning to adulthood with support in acquiring, retaining and improving self-help, socialization and adaptive skills necessary to be successful in the domains of employment, housing, education, and community life and to reside successfully in home and community settings. Independent living/skills-building activities are provided in partnership with young children to help the child/youth arrange for the services they need to become employed, access transportation, housing, and continuing education. Services are individualized according to each youth’s strengths, interests, skills, goals and are included on an individualized transition plan (i.e., waiver POC)
