Our Care Pathway is developed with an enablement focus in the delivery of care, to enable Service Users to meet their own needs wherever possible promoting independence and empowering Users to apprehend the outcomes within their care plan. However, reducing reliance upon services as far as possible and promoting independence. Our Service will put the service user on the centre of their Care Planning so that we can achieve person Centred values and objectives.
