Our service offers a wide range of service from personal care, practical tasks, shopping, social inclusion, escorting to GP/Hospital Appointments, Respite Care, Companionship, Dementia Care, 24 hour Live- In Complex Care, Palliative Care, End of Life Care.
Our team of carers have different kinds of specialism to assist all your caring needs. We have single support to double up teams to safely support your moving and handling needs.
My team members have either completed their Care Certificate Training or working on completion. Also, we support their development by offering them further training to enhance their skills and knowledge. They have completed their Induction Program and are very familiar with what is expected of them including the AM2PM Quality Care Limited Policies and Procedure for their guidance.
To ensure that all calls are covered we have a call monitoring system that we check regularly on a daily basis to ensure that all clients calls were serviced and that any concerns raised have been actioned by communicating with the entire team and document everything. Communication is crucial hence we make sure that everyone involved is updated and fully aware of the changes. Involvement of our Senior staff ie Field Care Supervisor and/or Care Coordinator in the community is paramount for proper handover is given to the team to ensure continuity of care is achieved at all times may it be changes on client's condition, medication, call time, diet and/or anything that is important to our client's well-being.
Completion of all needed Assessments is crucial as we need to use it as our baseline assessment and base our Care Plan on the initial meeting hence it is imperative that we attend/request for Client's Care Review depending on the situation to ensure that the proper Care Provision for our service user is given so involved with the multi-disciplinary team will be best honoured and adhered to. Ensuring client's safety is paramount in our delivery of service and ensuring that we are safeguarding each and every one of them from any potential harm/abuse is dealt with utmost importance.
