Starting a new package of care and bringing all our expertise to bare to respond to the need, and seeing that positive change which has improved the lives of the client and those closest to them. Relatives find it a huge relief that someone they can trust can meet issues head on and deal with problems with medications or accessing health service or equipment.

My Mother In Law. She lived close to us and had dementia and a series of complex health needs. My wife was coordinating all the appointments, medications etc and it became a full time job which started to affect her health and wellbeing. Care companies were only interested in 'doing the care' and not anything else. That demonstrated that there was a need for someone to provide much, much more than just normal home care. So we brought in our combined 40+ years care and nursing experience and started Liberty Private Care with the ethos of redefining private home care.

We do much more than traditional home care organisations by providing nurse led advanced care services to our clients, supported by the enhanced registration we have with the Care Quality Commission and external accreditations like the Six Steps End of Life Award.

We work on the basis of client first and so a client sees a regular small number of carers to be their care team.

Whilst hands on care is done on a person to person basis, access to our registered nurse and registered manager can also be done via the telephone or various media.

All our carers are trained in infection prevention and control and have that knowledge reviewed several times through the year. They also have free access to PPE as they need including masks, gloves, aprons, and hand sanitiser. We also support our carers by providing hand moisturiser to promote good hand conditioning. Our nurse carries out infection control audits to ensure that all measures that can be taken to reduce or eliminate the risk of any infection, not just COVID-19, are implemented.

Services

We provide a range of care needs to people in their own homes. This includes care to people whom have physical, health, mental health difficulties, needing help and support with washing, dressing, personal care, administering medications, meals and drinks, outings and supporting people with appointments. These issues may have been caused by dementia, stroke, age related problems, head or spinal injury or specific conditions such as MS or Parkinsons. Even though the service is provided for the client themselves our service is also designed to support those closest to the client, especially if they live a distance away or have busy professional and home lives themselves.

These services can range from simple day to day support with one carer visiting each day, to several visits each day with any equipment needed. Being led by a registered nurse, and having additional registrations with the Care Quality Commission, means that we can meet greater need and work at a significantly higher level that most other care providers can. These skills are especially valued when a client may need more complex care.

There is no ‘one size fits all’ scenario as every client has their own needs.

We only employ carers with several years of direct hands on experience and formal qualifications in care, and support them to develop their skills and capabilities by providing ongoing professional development with the clinical oversight and support provided by an experienced registered nurse and registered manager.

As a person moves towards the end of their life, they may experience the need for more bespoke and individual care services according to their and their family’s needs. This can include support with getting ready for the day or more comprehensive support including help with medication, personal care and nursing support and working with other health professionals.

We have received the Six Steps End of Life accreditation through the Countesss Mountbatten Hospice in Southampton, this means that the client and those closest to them can rely on a service which has met a set of nationaly standards independntly assessed by the Hospice and recognised by national bodies such as the NHS, Cancer Networks and Clinical Commissioning Groups.

All home care providers must be registered with the Care Quality Commission (CQC) to deliver personal care. (A ‘Regulated Activity’) The greater majority of home care providers have a limit to the range of services they can provide as carers. At Liberty Private Care we have an additional registration with the CQC which enables us to provide enhanced services. This in tandem with the fact that we employ a Registered General Nurse as our Nurse Care Manager means that we can bridge the gap between care and nursing and deliver advanced care beyond current limiting traditional models.

Our Nurse trains and mentors all our carers to provide enhanced services and example of that is the ability to take clinical observations to monitor a client’s health. This involves taking a set of observations in your own home including their blood pressure, heart rate, oxygen saturation and temperature. These observations are reviewed by our registered nurse can be relayed to your clinician for further evaluation. Early identification of an issue promotes early access to health services and early recovery.

We have a car which is wheelchair accessible, to enable clients to go on social outings or to health appointments accompanied by one of our carers. Often people cannot rely or use public transport and could not get to an appointment unless accompanied. This creates huge problems for clients and their families who may live a distance away or simply cannot get time from work. So we introduced this service and made it available.
